Patient referral is easy:
NON DENTAL REFERRALS:
We accept referral letters on headed paper that contain the following information: referrer contact details, patient's name and date of birth, the area of interest and the type and purpose of examination. If you have a specific question for the radiologist, please be as precise as possible and always give the patient's medical history.
DENTAL REFERRALS:
Simply log onto our Web-User page where the on-line referral forms can be filled in; registration and use of this on-line service is completely free.
Alternatively, please download the referral form (PDF) for the Cavendish Imaging Centre you wish to use (Harley Street referral form, Finchley referral form, Birmingham referral form , Oxford referral form) and return it to us by email, fax or post. Nottingham referrals are online only. However if you do prefer a paper referral form, just contact us!

Please also indicate whether you require a Radiology Report or will make your own Reporting arrangements.
